LGD-4033 commonly also referred to as ligandrol is an investigational selective androgen receptor modulator (SARM) used in the treatment of muscle wasting, muscular dystrophy, osteoporosis and other similar conditions. It was discovered by Ligand Pharmaceuticals based in San Diego and is currently being developed by Viking Therapeutics.
LGD-4033 and other SARMs compounds has been subjected to a lot of clinical research in the past two decades. LGD-4033 binds to androgen receptors with great affinity and precision, this simply means that it binds in a strong and targeted manner. Although LGD-4033 is compatible with most of the receptors, it isn’t compatible receptors found along the eyes and other areas with high androgen density.
Ligandrol is is best known for the anabolic benefits it has on bones, muscles, and prostate glands.
Studies have revealed that LGD-4034 is expected to offer the same therapeutic benefits as testosterone but with improved tolerability and acceptance by patients.
Structure of LGD-4033
Studies on animal based subjects revealed the compound was well tolerated. There was no result of serious adverse effects related to the use of the compound. Frequency of adverse events was almost identical when compared in active and placebo groups. There was no significant change in hemoglobin, prostate-specific antigen, aspartate aminotransferase, alanine aminotransferase, or QT intervals at different concentrations of administration. Research indicated that the compound had a long elimination half-life and dose-proportional accumulation when multiple doses were administered to animal subjects. LGD-4033 administration was linked with changes such as; dose-dependent suppression of total testosterone, sex hormone–binding globulin, high density lipoprotein cholesterol, and triglyceride levels. Administration of little quantities of the compound caused a significant suppression of follicle-stimulating hormone and free testosterone. There was also a visible and significant increase in lean body mass, although fat mass did not change significantly. Hormone levels and lipids returned to baseline after administration of the compound was ceased.
There was conclusive evidence of the compound’s androgenic activity, as indicated by the increase in LBM, notable suppression of testosterone, sex hormone–binding globulin, and HDL cholesterol levels even when the compound was used over a short period. Despite the demonstrable androgenic activity, serum prostate-specific antigen was not significantly affected.
Ligandrol has numerous attractive PK features. Because of its long elimination half life, it can be administered once daily or even less frequently. Daily administration of the compound resulted in dose-proportional increases in systemic exposure which led to predictable accumulation upon multiple dosing.
As with all androgens, LGD-4033 was associated with significant suppression of HDL cholesterol. There was also a decrease in triglyceride levels, although LDL cholesterol was unaffected. In animal tests, the amount of anti-atherogenic effect of HDL cholesterol is influenced more by the mechanism of HDL modification than by the changes in HDL levels. In conclusion, the significance of the HDL decrease associated with oral androgens isn’t completely clear. Longer researches are required to shed more light on the cardiovascular risks associated with long-term SARM administration. For now, the first trials are likely to be centered on acute or subacute indications like cancer cachexia and functional limitations linked with acute illness or hip fracture, where the short-term changes in HDL cholesterol may not be of clinical use.
